Jakub Slavík
Jakub Slavík (born December 6, 1993) is a Czech ice hockey player. He has played for the HC Plzen, making two professional appearances. While making only one Extraliga appearance in 2013-14, he spent the season on loan in the lower Czech leagues.[1]
